Welcome to Chester Central Primary Care Network

Chester Central PCN is a collaboration between six GP practices across central Chester with a combined population of 38,962.

What is a primary care network?

Primary Care Networks (PCN) are a key part of the NHS Long Term Plan. They consist of groups of general practices working together with local providers such as community services, social care and the voluntary sector, to respond to the needs of the patients they serve.

The network aims to focus on population health and addressing health inequalities in their local area, providing a more personalised, coordinated and social care services to their local population. Learn more

How could this affect you?

Whilst patients won’t see any sudden changes, over time they will see a range of new clinical services available in their local GP surgery.

You may be offered an appointment or be seen by a range of different health professionals which your GP feels are best able to meet your needs.

You may see a greater focus on prevention with more attention on community-based activities that will help people live healthier lives.
